Job Description:
Some of the accountabilities for this role are:

-Supervises and leads the CPP in accordance with current Community Paramedicine standards, regulations and Corporate policies.
-Supervises assigned staff including staff development, performance management/review, coaching/mentoring, health & safety, collective agreement administration, discipline, and termination decisions.
-Supports existing health services and provides integrated health services in partnership with other health professionals and organizations.
-Extends access to health services delivery in high risk populations, including primary care, public health, disease management, prevention, wellness, and mental health.
-Ensures that CPPs align with the Community Paramedicine framework as outlined by the Ministry of Health and Long Term Care (MOHLTC) and the Northwest LHIN.
-Develops and implements new initiatives, pilots and programs to assist in improving the health status of targeted residents as approved by Senior Management.
-Oversees data collection and analysis pertaining to community paramedicine activities, for program evaluation and quality assurance.
-Provides detailed qualitative and quantitative reporting on community paramedicine activities to City Council, LHIN, MOHLTC, etc.
-Engages the community through initiatives, programs and educational presentations, including diabetes prevention/treatment, hypertension, falls assessment, injury evaluation, geriatric frailty visits, and nutrition.
